How To Qualify:
1. Reach out to us through the "Nomination" page, or email us directly at christinesfund@gmail.com with your pet's story.
2. Schedule the veterinary appointment, and provide us with that information. Be sure to include the name of the clinic, the veterinarian you are scheduled with, and the time and date of the appointment.
Any contributions made by Christine's Fund will be paid directly to the veterinary clinic. We do not reimburse veterinary expenses to the pet owner.
3. Provide documentation that shows you have applied for but do not qualify for CareCredit.
If the veterinary clinic you are using offers alternative payment methods other than CareCredit, like ScratchPay, AllPet, Cherry, or SunbitNow, we may also ask for proof that you applied for but do not qualify for those alternative options.
4. If your request is urgent, please state this is the email subject line, so we can prioritize your application for assistance.

1. While Midtown Animal Clinic is our provider of choice, you are welcome to use any veterinary clinic you prefer.
2. All contributions require prior authorization. Please do not attend your appointment at Midtown Animal Clinic, or any other veterinary facility, with the expectation of funds being available unless you have received prior authorization from our foundation.​
